Prior to your discharge, a plan will be developed that determines any equipment needs and in-home services you may require. John Knox Village home health services are available to help with your transition – providing physical, occupational and speech therapies in the comfort and privacy of your own home. You may also visit Oak Park Outpatient Services to continue rehabilitation and outpatient therapy services.
